Texas CARES Volunteer Self-Certification Policy

1. All Volunteers should complete a self-certification prior to any in person volunteer activity in the Texas CARES Adoption Center or other Texas CARES event where they will be in contact with others.
   
   
2. If the answer is going to be “yes” affirming the presence to any of the following questions, do not report for volunteer work in person and instead let the volunteer coordinator know you will be absent. There is no need to fill this form out if you are going to mark “yes or affirm” for any of the questions.
